Excavation Permit Instructions
For driveway construction, review the Standard Concrete Driveway drawings before proceeding any further.
- Open the Right-of-Way Permit form and fill it out online. (there are additional instruction below)
- Make a sketch drawing of the property showing a North Arrow, Street Name, Address, and the location of the proposed work.
- Mail both the sketch and permit application to:

Instructions for filling out the right-of-way permit application
Application made by Enter the name of the individual the permit is to be issued to.
Address Enter the address of the above named individual the permit is to be issued to.
Telephone Enter the telephone number of the above named individual.
Fax Enter the fax number for the above named individual.
Contact person Enter the name of the individual to contact when the permit is ready to be picked up. If it is the same individual as above, then enter "same".
For a permit to Enter the type of work to be preformed (i.e.: install new drive, replace existing drive, etc.)
Location Enter the address or legal description of the location where the above permitted work will be preformed.
To be completed on or before Enter a completion date (allow 90 days for work to be performed).
Application fee $10.00 per location (circle drive - $20.00).
PROPERTY OWNER If you are the property owner, then sign and date on the provided line.
CONTRACTOR If you are the contractor, then sign and date on the provided line.
